Iron is the mineral your body uses to make haemoglobin — the protein in red blood cells that carries oxygen from your lungs to every part of the body. When iron runs low, less oxygen reaches your muscles and brain, and you feel it as tiredness long before you think of it as a health problem.
Left untreated, iron deficiency can progress to iron-deficiency anaemia. It is one of the most common nutritional problems in India: the National Family Health Survey (NFHS-5) found that more than half of Indian women aged 15–49 are anaemic. Men get it too, but women are affected far more often.
Why it happens
- Heavy periods: regular monthly blood loss is the most common reason in women.
- Pregnancy: the body's need for iron rises sharply to support the baby.
- Not enough iron in the diet, especially in diets low in pulses, greens and other iron sources.
- Blood loss from other causes, such as stomach ulcers, piles or intestinal worms.
- Digestive problems that reduce how much iron the gut can absorb.
Symptoms to watch for
Early on, low iron is easy to dismiss as ordinary tiredness. As it worsens, common signs include:
- Constant tiredness, weakness and low energy
- Breathlessness or chest discomfort on climbing stairs or light activity
- Headaches and difficulty concentrating
- Irritability
- Pale skin, inner eyelids or nails
- Unusual cravings for non-food things such as ice, raw rice, chalk or mud
Symptoms alone can't confirm iron deficiency — thyroid problems, vitamin B12 deficiency, diabetes and many other conditions can feel the same. A simple blood test (haemoglobin, and often ferritin) gives the answer. As a guide, the WHO treats haemoglobin below 12 g/dL in women, 13 g/dL in men and 11 g/dL in pregnancy as anaemia.
Iron-rich foods you'll find in a Tamil Nadu kitchen
| Food group | Everyday examples |
|---|---|
| Greens | Murungai keerai (drumstick leaves), arai keerai, siru keerai, mulai keerai |
| Millets | Kambu (pearl millet), ragi |
| Pulses and legumes | Kollu (horse gram), kondakadalai (black chana), rajma, green gram, lentils, peas |
| Seeds and nuts | Ellu (sesame) — try ellu urundai — groundnuts, almonds, sunflower seeds |
| Dried fruit | Dates, raisins, dried figs |
| Non-vegetarian | Mutton, liver, chicken, eggs, fish — small fish like nethili are a good choice |
| Fortified foods | Iron-fortified cereals and fortified rice where available |
Iron from meat, fish and poultry is absorbed more easily than iron from plant foods. Vegetarians can still meet their needs — it just helps to combine foods well.
How to absorb more iron from your meals
- Pair iron with vitamin C: a squeeze of lemon over keerai or sundal, a nellikai (amla), guava or orange after a meal, or tomatoes in your curry all help the body absorb plant iron.
- Keep tea and coffee away from meals: they reduce iron absorption. Have them at least an hour before or after eating.
- Don't take calcium tablets at the same time as an iron-rich meal or iron tablets — space them apart.
- Cut down on fast food and packaged snacks that fill you up without adding iron.
When to see a doctor
If tiredness, breathlessness or the other signs above keep coming back, see a doctor and get your blood tested. Pregnant women, girls who have started their periods and anyone with very heavy periods should have their haemoglobin checked regularly. Government primary health centres provide iron and folic acid tablets free to pregnant women and adolescents under the national anaemia programme.
Don't start iron tablets on your own. The cause of the deficiency — such as hidden blood loss — needs to be found and treated, and too much iron can be harmful. Always keep iron tablets out of the reach of children: an accidental overdose can be dangerous for a young child.
