Madurai's “winter” is gentle compared with north India, but parents know the pattern well: from the northeast monsoon in October through the misty (pani) mornings of December to February, the coughing starts. One child brings a cold home from school, and within a week the whole house is up at night. Most of these coughs are harmless and pass on their own — the key is knowing how to keep your child comfortable, and which signs mean it is time to see a doctor.

Why coughs rise in Madurai's cool season

  • More viruses in circulation: the common-cold and flu viruses spread easily in classrooms, school vans and autos, where children sit close together for long periods.
  • Damp monsoon weeks: wet clothes, damp walls and mould in closed rooms can irritate sensitive airways.
  • Cool morning air: early two-wheeler rides to school through the morning mist can set off coughing in children with sensitive airways.
  • Smoke: Bhogi bonfires in January, mosquito coils, incense, camphor and anyone smoking at home or nearby all irritate a child's lungs.
  • Dust: road and construction dust, and blankets and mattresses brought out of storage for the cooler months.
  • Small airways: a child's air passages are narrow, so even a little swelling or mucus triggers a cough. Coughing itself is useful — it is how the body clears the airway.

What kind of cough is it?

Listening to the cough, and noting when it happens, tells you — and your doctor — a lot. Keep a simple note of how many days it has lasted, whether it is worse at night and whether there is fever.

Home care that actually helps

  • Keep fluids going: warm water, clear soups or light rasam, tender coconut water, and breastfeeding for babies. Well-hydrated children bring up mucus more easily.
  • Honey for children over one year: half to one teaspoon before bed can ease a night-time cough. Never give honey to a baby under 12 months.
  • Saline nose drops can loosen a blocked nose, especially before feeds and sleep in young children — ask your pharmacist or doctor which to use.
  • Steam, safely: let your child sit with you in a bathroom filled with steam from a hot shower. Avoid bowls of boiling water — scald burns in children are common and serious.
  • Rest and sleep matter more than any tonic. Keep them home from school while they have fever.
  • Keep the air clean: no smoking indoors or near the child, and pause mosquito coils, incense and sambrani smoke while they are coughing. Use nets or plug-in repellents instead.

What to avoid

  • Cough syrups and cold medicines without a doctor's advice — many are not suitable for young children, and India's drug regulator has specifically warned against certain cold-medicine combinations for children under four.
  • Leftover antibiotics or a sibling's prescription. Most winter coughs are viral, and antibiotics do not help them.
  • Camphor and menthol rubs on children under two, and never near a baby's nose or mouth.
  • Adult medicines or adult doses, even “just half a tablet”.

Warning signs: when to see a doctor

Go to a doctor or hospital immediately (call 108 for an ambulance) if your child: is breathing very fast or struggling to breathe; has the skin between the ribs sucking in with each breath; makes a harsh noise while breathing at rest; has bluish lips or face; cannot drink or breastfeed, or vomits everything; is unusually drowsy or hard to wake; has a fit; or coughs up blood.

As a rough guide from the World Health Organization, breathing is too fast if a child at rest takes 60 or more breaths a minute under 2 months of age, 50 or more between 2 and 12 months, and 40 or more between 1 and 5 years. Count for a full minute while the child is calm.

See a doctor within a day or two if: a baby under three months has any fever; fever lasts more than three days or keeps coming back; there is ear pain or pus from the ear; the child is not eating or seems to be getting worse instead of better; or the cough comes in long fits with vomiting.

A cough that lasts more than two weeks should always be checked. Your doctor may look for asthma, allergy, sinus problems or — because it is still common in India — tuberculosis, especially if there is weight loss or someone in the household has a long-standing cough.

Preventing the next round

  • Keep vaccinations up to date. The free Universal Immunisation Programme covers diseases that cause serious coughs, such as whooping cough, measles and pneumococcal and Hib infections. Ask your doctor whether a yearly flu vaccine is right for your child.
  • Handwashing with soap after school, before meals and after blowing the nose.
  • Teach children to cough or sneeze into their elbow, not their hands.
  • Keep a sick child home from school for a day or two — it protects classmates and speeds recovery.
  • Dress in layers for early-morning rides, and dry out damp rooms and clothes during the rains.
  • Keep your home smoke-free all year, not just when someone is ill.
